The Student Residency Questionnaire is for students and families experiencing homelessness, temporary housing, or housing instability. This form helps determine eligibility for support and services under the McKinney-Vento Homeless Assistance Act.

If you believe you are eligible, please complete the form in English or Spanish. Once completed, email the form to the McKinney-Vento District Liaison, Amanda McAdams, at Amanda.Mcadams@guhsdaz.org, or submit it to the school social worker at your student’s campus.
